One of the costliest problems in health care today is that patients fail to follow caregiver directions between office visits.
San Rafael, Calif.-based Conversa Health operates a patient relationship management platform to help providers deal with this problem. The technology enables “continuous, collaborative, structured communications” between caregivers and patients, Conversa says.
Conversa’s platform sends personalized messages to patients based on a patient profile derived from electronic health records, patients’ biometric devices, and patient-reported data.
The company says it has now closed a $2.5 million seed round with a group of unnamed health care angel investors.

Conversa will use the new funds to expand its service through distribution partners in care management, population health, and customer relationship management.
“Patient relationship management is an exciting new category of health care solutions that addresses patient engagement and population health management,” said Conversa CEO West Shell III.
